This course aims to provide a deep understanding of process accidents, their assessment and monitoring, their modeling and integration with risk assessment, and more importantly their use in safety barrier design and accident prevention. Further, as plant operate and generate new data related to incident and near misses it may be used in updating the likelihood of any potential accident occurrence and help to revise risk factor (which is important concept called learning from incident and near misses). The revised risk factor could further be used for risk/safety management. This course cover all these topics with case studies
